Prescribing Sleep Assessment

Indications, primary complaint, diagnosis and suggested studies

Primary Complaint – Suggested Study Patient has one or more of the following indications Most common diagnosis

EXCESSIVE SLEEPINESS = Diagnostic Study

(symptoms are general, need to re-evaluate after diagnostic results available)

  • No energy/ feel worn out, depressed, irritable or overwhelmed
  • Trouble remembering things/ worry about memory/ feel less efficient than they should be
  • Dozed off in meetings, at work, during movies or while spending time with others
  • While driving, have run off onto the berm or have "driven on autopilot"
  • Trouble concentrating or have been suspected to have attention deficit disorder
  • Still tired or groggy when they get up from sleep

Unspecified Sleep Apnea, Hypersomnia NOS, Narcolepsy without cataplexy, Idiopathic Hypersomnia

SLEEP APNEA = Split Night Study

(most common symptoms occurring, may be able to achieve diagnostic/therapeutic study in one night)

  • Awakened with one or more of the following: snores, snorts, choking, gasping, shortness of breath, chest pains, palpitations, anxiety, dry throat, nasal congestion or headaches
  • Night sweats, restless sleep, falling out of bed, bedwetting or awakenings to urinate
  • Others say that the person snores or stops breathing
  • Muscle stiffness/ soreness or a diagnosis of fibromyalgia
  • One or more of the following apply:
    • Person is overweight
    • Has high blood pressure
    • History of heart trouble
    • Is post-menopausal
  • One or more of the person's blood relatives suffers from:
    • Heavy snoring
    • Sleepiness
    • Diagnosed sleep apnea
  • The person has scoliosis, muscle weakness, neuromuscular or chronic lung disease

Unspecified Sleep Apnea, Hypersomnia NOS, Narcolepsy without cataplexy, Idiopathic Hypersomnia

NARCOLEPSY/HYPERSOMNIA = MSLT Study

(preceding night study will determine if other problems occurring)

  • Sees or hears things when they're drowsy, seems to begin dreaming before they're fully asleep
  • Experienced sudden muscle weakness with laughter, anger, pleasurable events or other emotions
  • Often dreams right away when they doze off or seem to dream intensely while they sleep
  • Experienced paralysis or inability to move when awakening or when dozing off

Narcolepsy, Transient/Persistent Hypersomnia, Sleep Paralysis – recurrent isolated

GERD = Diagnostic Study

(determine if sleep apnea is inducing or aggravating GE reflux)

  • Awakenings with a sour or bitter taste, vomiting, wheezing, chest pain, heartburn, choking or dry cough
  • Diagnosed with chronic esophagitis or Barrett's esophagus

Obstructive Sleep Apnea, Unspecified Sleep Apnea, Organic Sleep Apnea NOS

INSOMNIA = Diagnostic Study

(determine if other problems occurring)

  • Often can't fall asleep for some time
  • Symptoms of Restless Legs Syndrome: their legs feel "antsy"; they feel like they must keep moving their legs to get relief
  • Wakes up frequently, with or without any associated symptoms
  • Often wakes up too early and has trouble returning to sleep
  • Sleep is light, fragmented or un-refreshing
  • Frequently taken something to help them sleep

Transient/Persistent Insomnia, Insomnia NOS, Organic Insomnia NOS/NEC, Insomnia with Sleep Apnea NOS, Periodic Limb Movement Disorder, Sleep-related Leg Cramps
